Has anyone experienced problems with aminoglycoside/antibiotic overdosing?<

By the way, the intensivist was the one to order the amikacin blood level test, buy my father was only in the critical care unit for a couple days. Thus who was supposed to take over from there? Pharmacist or the primary care?
